Family Communication: Keeping your family informed about any changes in your care needs is essential. Your Senior Carer will communicate effectively with your family members, providing updates on your condition and involving them in decision-making processes regarding your care.

Your Senior Carer will ensure that all changes to your care plan are accurately recorded in our record-keeping app, including updates to your care needs, assessments, and communication with the multidisciplinary team and family members.

This allows for timely review and adjustment of your care package, including coordination with relevant authorities such as Continuing Healthcare (CHC) or social services if necessary.
